Instructions for Obtaining a Working Permit

*Please note: Anyone under the age of 18 who is employed or planning to become employed, must obtain a working permit.*

  1. A parent/guardian must appear in person with the student at Elizabeth Forward Senior High School Office and sign the bottom front of the PDE-4565 Working Permit Application. In lieu of a personal appearance, a parent/guardian may execute a statement before a Notary Public or other person authorized to administer oaths attesting to the accuracy of facts set forth on the application.(a form for that purpose is available at the bottom of this page as well as at the Senior HIgh School).
  2. Have the employer complete the top half of the back side of the application.
  3. Have your doctor complete the bottom half of the back side of the application. If you have had a physical at school in the past year, you can have the nurse make a copy of your health record instead of taking another physical(only when school is in session).
  4. If you haven't already done so when applying for your working permit, bring in some verification of your date of birth such as a birth certificate, baptismal certificate, driver's license, learner's permit, passport, etc.
  5. The application must be returned in person must witness your signature on your new working permit. The school official must witness you signing the application.
  6. Hours of application are between 8:00 a.m. and 3:00 p.m. on Monday through Friday at the Elizabeth Forward High School Office.
