Employment Guidelines for Teachers
The following items should be completed and returned to the personnel office at the district office when seeking teaching employment with the Elizabeth Forward School District:
1) Application for employment (we accept the standard teaching application available at http://www.state.pa.us
2) A copy of your teaching certificate (if applicable).
3) A copy of your Criminal Record Check Clearance (Act 34).
4) A copy of your Child Abuse History Clearance (Act 151).
5.) A copy of your FBI Federal Criminal History Clearance (Act 114).
Registration is completed online www.pa.cogentid.com Telephonic registration is available at 1-888-439-2486 Monday through Friday, 8am to 6pm EST.
6.) A copy of your transcripts.
7.) If you wish, you may attach your resume, letters of recommendation, and a letter of intent to your application.
If you would like to be placed on our substitute list, please check the appropriate space on the bottom of the front page of the application. An interview with one of our principals will be arranged. Following the interview, your name will be submitted to the board for approval. After the board meeting, the following paperwork should be completed at the district office.
1. Employment Eligibility Verification Form (I-9)- Provided at the district office.
2. Driver's License and Social Security Card- We need to see the originals which we will photocopy for our records.
3. W-4 Form for Income Tax Purposes- Provided at the district office.
4. Payroll Information- Provided at the district oriface.
5. Tine Tests- We will need to see proof of a negative tine test
If you are hired on a fulltime basis, the superintendent's office will request to see your original teaching certificate, transcripts, clearances, and a complete physical will be required.
