1.If you will be going to a private physician for your physical pick up a code of conduct and a comprehensive physical form – Sections 1-4
2. All other athletes will need the comprehensive physical form (Section 1 to 4) filled out by their parents before the school doctor will give them a physical. Bring the form with you on one of the days of the physicals or turn it in to the nurse or the athletic secretary before school is out.We will hold the papers for you.
3.The Code of Conduct Packets are returned to your coach on the first day of practice. You cannot practice without a completed code ofconduct and a physical.
4.Free sports physicals will be given onTuesday, July 27th and Tuesday, August 3rdfrom 8:30 a.m. to 12:30 p.m. in the High School Nurse's Office.
5.You cannot have a physical for the 2010-11 season before June 1st, 2010.
The forms must be filled out in pen by the parent/guardian (not the student) or they will be rejected.
If a player will be participating in a Winter and or Spring sport after having the fall physical (Section 1-4), they must fill out Section 5 and return it to the nurse or the athletic office. If a player is injured they will need a release from their doctor andSections 5 - 6 filled out.
Winter and Spring sports participants can also have their physicals taken in the summer if they so choose. They will still need a Section 5 at the start of their season.
Wrestlers will still need to have their minimum weight classification (Section 7) filled out and returned to Mr. Guffey, the nurse or the athletic secretary.

All student athletes must have a physical and insurance before participating in any try-out or practice.
